Swipe a task to the left to view the comment icon:

Click on the word balloon icon to enable comments to be entered for that task. Click "Save" when the comment has been entered.
Click on the "Mountains" icon to take a picture. Note for Android phones: if this doesn't show the camera immediately, you made need to allow permissions for StakTask to allow both Camera and Storage.
